End User Manager: The QIMS Account Approval Process (4/17/12 @ 2pm-3pm ET)
End User Managers (EUMs) serve as the first level of authority in QIMS. EUMs are responsible for reviewing and approving QIMS account requests, and it is important that EUMs understand the steps required to perform these tasks. To assist EUMs in this area, CMS and the CROWNWeb Outreach, Communication, and Training (OCT) Team will walk EUMs through the steps that they must follow after they receive an applicant’s request for access to QIMS and CROWNWeb.
This session is for anyone serving as an End User Manager. A separate event is being held for Security Officials on Thursday, April 19, 2012. See below.

Security Official: The QIMS Account Activation Process (4/19/12 @ 2pm-3pm ET)
Security Officials (SOs) serve in the top-level authority role in QIMS. During the QIMS account registration process, an SO can review and activate an applicant’s account request once it is received from the EUM. To assist SOs with understanding how to activate applicants’ accounts in QIMS, CMS and the CROWNWeb Outreach, Communication and Training (OCT) Team will walk SOs through the steps that they must complete to correctly activate users’ QIMS accounts.
This session is for anyone serving as a Security Official. A separate event is being held for End User Managers on Tuesday, April 17, 2012.

CROWNWeb At First Glance – Accessing and Understanding CROWNWeb (3/29/12 @ 2pm-3pm ET)
With CROWNWeb’s National Release approaching, new CROWNWeb users will need to know how to access the system, how to log in, and what to look for to ensure that their accounts are ready for use. To help users obtain this understanding, the Centers for Medicare & Medicaid Services (CMS) and the CROWNWeb Outreach, Communication, and Training (OCT) Team will discuss and demonstrate data entry in the various sections of the data-collection system. This session will highlight how you can ensure that your role(s) and scope were assigned correctly, as well as data entry expectations for CROWNWeb’s Facilities, Personnel, Patients, Reports, Clinical, and Form 2744 tabs.

Mike was a Board of Nephrology Examiners Nursing and Technology (BONENT) Certified Hemodialysis Technician (CHT) for 10 years and conducted clinical in-services in hemodialysis clinics for a major dialysis manufacturer. He develops eLearning materials for the Project CROWNWeb website.
Michelle Barry Michelle Barry is the Technical Writer for the CROWNWeb Outreach, Communication, and Training Team. Since joining the CROWNWeb team in late 2010, Michelle has been developing the training materials for the CROWNWeb project. Addressing Your QIMS Questions (2/23/12 @ 2pm-3pm ET)
What is the difference between a Security Official and an End User Manager? Why do Security Officials receive two account IDs? Why do they NEED two account IDs? How does the QualityNet Help Desk manage account requests and technical issues?
CROWNWeb's National Release is right around the corner. However, questions continue to arise about the QualityNet Identity Management System (QIMS) account registration process. To help answer these questions, the Centers for Medicare and Medicaid Services (CMS), the CROWNWeb Outreach, Communication, and Training (OCT) team, and the QualityNet Help Desk will address some of the most frequently asked QIMS questions. Additionally, this call will provide a detailed walkthrough of the registration process for an initial Security Official (SO) and initial End User Manager (EUM). Amie DeForest Amie has worked on the QualityNet Help Desk for three years. In her 11+ years working with the Standard Data Processing System (SDPS) community, Amie has provided training and support for the Quality Improvement Organizations as well as hospital and physician office initiatives.
National Release QIMS Walkthrough (1/26/2012 @ 2pm - 3pm ET)
With CROWNWeb's National Release approaching, users who will need to work in CROWNWeb must first set up their accounts in the QIMS system - the QualityNet Identity Management System. Exploring an HIE Solution for Small Dialysis Facilities (11/30/2011 @ 2pm - 3pm ET)
Since CROWNWeb’s initial release, CMS has worked with three large dialysis organizations and their corporate offices to test the ability to transfer data directly into CROWNWeb by using an electronic data interchange (EDI) format. Determined to find an electronic data submission solution for small- and medium-sized dialysis organizations, CMS has been working with the National Renal Administrators Association (NRAA) to test a Health Information Exchange (HIE) technology that will submit data via a Nationwide Health Information Network (NwHIN) gateway. Join CMS and Howard Thomas, the NRAA’s HIE Project Manager, as they discuss the NwHIN project’s background, status, and timeline.
Howard Thomas is an independent consultant working as the overall project manager for the NRAA HIE project. Howard works as a consultant on numerous technology and business projects and has led the NRAA's efforts in this area since October 2010. Howard has also served as an HIE technology consultant for Washington State and worked closely with many other HIE enterprises, claims clearing houses, and related data management organizations. Howard holds an Industrial Engineering Degree from Louisiana State University and an MBA from St. Mary's College in the San Francisco Bay Area.

As Phase II Expansion continues, hundreds of facilities encompassing a wide spectrum of CROWNWeb users are currently entering data into the system, and the experiences one facility has while learning to use CROWNWeb may differ dramatically from that of another facility.

Jeff Arnall has 10 years of data management experience in the ESRD community. In his current role as Director of Information Systems for Heartland Kidney Network (Network 12), he oversees ESRD healthcare information activities for Missouri, Kansas, Iowa and Nebraska.
Jeff is a member of the Project Management Institute and a certified Project Management Professional (PMP) with 19+ years of experience in the healthcare community with varied responsibilities in physician credentialing, medical software helpdesk senior support, systems analyst and Java programmer.
Terri Day is an Administrative Secretary for Mercy Plaza Dialysis. Mercy is a hospital-based independent facility that opened its first dialysis unit in 1985; and started its home-based peritoneal dialysis program in 1987. Today, Mercy has three facilities, 47 stations, and treats nearly 200 patients. Mercy has approximately 60 employees, of which three are CROWNWeb users.
Mary Youngblood is an Executive Assistant for Affiliated Hospitals Dialysis Centers (AHDC). AHDC celebrated its 30-year anniversary on July 23, 2011. Once an independent, not-for-profit organization that was jointly owned by two area hospitals, AHDC was acquired by Ambulatory Services of America in July 2008.
AHDC currently has three operational units in the St. Louis area. It currently employs 66 individuals, and approximately half have received some level of CROWNWeb training. The majority of AHDC’s CROWNWeb data entry is managed by a dozen staff members ranging from Head Nurses, Charge Nurses, Unit Secretaries, and Executive Assistants.

Patricia S. Wilson is a Senior Data Analyst for 3M Health Information Systems. As team lead for the company's Healthcare Data Dictionary (HDD), Ms. Wilson is responsible for process flow, standard operating procedures, and job assignments. She is a member of the American Health Information Management Association (AHIMA), a voting member of International Health Terminology Standards Development Organisation (IHTSDO) Mapping Work Group, and member of the Logical Observation Identifiers Names and Codes (LOINC®®).
CROWNWeb's New Features – A First Look (1/27/2011 @ 2pm - 3pm ET)
With the next release of CROWNWeb on the horizon, questions surrounding the look and feel of the system's new features and functionalities have arisen. Join CMS for a first peek at the new screens and features that will debut in the upcoming CROWNWeb release, including the updated "My Reports" area, the new Time-Out functionality, and other changes designed to help improve the user experience.
Mike Moran is the Release Manager at Edaptive Systems for the CROWNWeb project. Mike has managed a wide range of software development projects for Federal and State government clients as well as commercial businesses over his 15+ years of experience. A majority of his experience has been with IT Service Management efforts supporting large scale service desks and support teams.

Tim Hesse is the Testing Manager for CROWN at Edaptive Systems. Tim has been executing, leading, and managing software test efforts for the past 14 years, of which nine have been in the ESRD community. Utilizing industry standards, Tim has implemented and maintained effective testing processes that prevent, detect, record, and manage defects within the context of a defined release.

CROWNWeb Data Discrepancy, Validation, and Reliability – A Look Behind the Scenes (12/21/2010 @ 2pm - 3pm ET)
A critical element essential to the success of CROWNWeb's full national release is the integrity of the data submitted into the system by users. Join CMS and representatives from Healthcare Management Solutions (HMS) and SoftDev Inc. as they discuss the processes in place to identify and correct data discrepancies, validate data from CROWNWeb end-users, and ensure the reliability of this data for all facility and government entity stakeholders.
Robin Kaiser is the Program Manager for the CROWNWeb Data Discrepancy Support Team at SoftDev Inc. In her role, Robin will be leading projects and activities to identify, resolve, and present solutions to prevent data discrepancies and data quality errors in the CROWNWeb system. Robin has worked on CMS ESRD systems for the past 10 years, including developing the SIMS, VISION, and Core Data Set applications and business analysis for the CROWNWeb system. She has over 14 years of experience in IT and data systems, and received her BS in Mathematics from Georgia Southwestern State University in 1997.
Chris Wright is the Project Manager for the CROWNWeb Data Validity and Reliability Team at Healthcare Management Solutions (HMS). As Project Manager, Chris is responsible for planning and executing all aspects of the data validation activities. Chris has over 7 years of experience in the IT field and has worked on other data validation projects with CMS in the past. He served as the Project Lead for the 2007 and 2008 Risk Adjustment Data Validation (RADV) projects. In this role, he was responsible for designing and implementing a medical record review abstraction tool, working with CMS to ensure that all security and data requirements were met, and supervising the work of all IT personnel involved in the project. Chris has worked very closely with government healthcare entities to develop new software systems which improve efficiency and streamline their processes through innovative, state of the art technology.
Open Door Forum: CROWNWeb 2.0 (Phase III) and Beyond (11/23/2010 @ 2pm - 3pm ET)
As the start of CROWNWeb 2.0 (Phase III) approaches, facilities continue to submit inquiries on the scope and impact of CROWNWeb 2.0 and the development of CROWNWeb 3.0, the full national release. Jacqueline (Jackie) Abt is a nursing information specialist with clinical nursing and management expertise. As a CMS central office employee, Jackie serves as an informatics liaison to the ESRD project officers/other CMS divisions on both core contract issues and the CROWNWeb data system.
Glenda Payne is a nephrology nurse with more than 30 years experience in ESRD. She has state and federal survey experience and is part of the Survey & Certification ESRD Support Team responsible for surveyor training and developing resources to assist in the implementation of the ESRD Conditions for Coverage. She serves as the project officer for the Network 7 contract, as well as for the RCT special project.

Jeff Arnall has 9 years of data management experience in the ESRD community. In his current role as Director of Information Systems for Heartland Kidney Network (Network 12), he oversees ESRD healthcare information activities for Missouri, Kansas, Iowa and Nebraska.
Jeff is a member of the Project Management Institute and a certified Project Management Professional (PMP) with 18+ years of experience in the healthcare community with varied responsibilities in physician credentialing, medical software helpdesk senior support, systems analyst and Java programmer.
Rhonda Lockett has been with the Network for 33 years (Network 22 for 11 years and Network 4 for 22 years as the Data Director.) In her role, Rhonda works with ESRD Network staff members regarding their data needs, and oversees the flow of data requirements, within Network 4, for the CROWNWeb program. Rhonda is responsible for maintaining an understanding of CMS modifications with regard to the Medicare non-reimbursement forms that must be completed by facilities. Additionally, Rhonda serves as the primary staff person for the Organ Procurement/Transplantation Committee.
Shane Perry has worked for Network 4 (the states of PA & DE) for 11 years as the Director of Information Systems. In his role, Shane is responsible for the design, development, implementation, and technical support of the Network 4 Information Systems, including conducting data analysis and completing special requests. Creates and maintains material found on the Network 4 website. Additionally, he coordinates Network 4's efforts in CROWN, i.e., Vital Information System to Improve Outcomes in Nephrology (VISION), REMIS, SIMS, QualityNet Exchange and CROWNWeb.
CROWNWeb: Better Data; Better Outcomes (9/30/2010 @ 2pm - 3pm ET)
Since its inception, the Clinical Performance Measures (CPM) Project has provided data and benchmarks to assist dialysis facilities in their goals of improving patient care and outcomes. In upcoming releases, CROWNWeb will expand its CPM data collection capabilities to incorporate new CPM calculations and provide expedited reporting capabilities. Join CMS and key stakeholders in the renal community as they discuss how CPM data and reports available within CROWNWeb may help facilities by providing data that could assist them in improving both facility performance and patient outcomes.
Tom Dudley has been with CMS since 2000. His duties include the development and public reporting of ESRD and Nursing Home clinical performance measures, and overseeing the development and implementation of MDS 3.0. He's worked on such projects as databases for the Prescription Drug Plan Finder and Online Enrollment Center, Dialysis Facility Compare, and the Medicare Eligibility Tool on medicare.gov. He holds a M.S. in Nursing and Business Administration and a B.S. in Nursing from University of Maryland, and has over 20 years experience in the planning, implementation and management of multidisciplinary health care programs.
Charlie Riesz has been with CMS since 2000 and is a Health Insurance Specialist in the Division of Chronic and Post Acute Care in the Office of Clinical Standards and Quality. His duties include the development of quality reporting programs (for long term care hospitals, inpatient rehabilitation facilities, and hospices) and public reporting of ESRD quality measures. Additionally, Charlie's professional background spans over 20 years in the public and nonprofit sectors with experience in planning, organizing, and managing complex analyses and projects. His education includes a M.P.A. in Public Administration and a B.A. in Political Science from The American University.

Jacqueline (Jackie) Abt is a nursing information specialist with clinical nursing and management expertise. As a CMS central office employee, Jackie serves as an informatics liaison to the ESRD project officers/other CMS divisions on both core contract issues and the CROWNWeb data system. She is also the program lead for CROWNWeb development, the GTL for NW 7's Renal – Requirements, Communications and Training (Renal RCT) contract, GTL for the Electronic Lab Data project, and Medicare Improvement for Patients and Providers projects.
The QIPS to QIMS Transition: “What You Need to Know” (7/29/2010 @ 2pm - 3pm ET)
CMS recently announced plans to launch the third phase of CROWNWeb in January 2011. However, while this release will introduce a new system for creating and managing CROWNWeb User IDs (the QualityNet Identity Management System – QIMS), potential users can still take advantage of the existing system (the QualityNet Identity Provisioning System – QIPS) for account creation. Join CMS for a QIPS refresher, an overview of the under-construction QIMS system, and a glance at Multi-Factor Authentication.
Rebecca Daniels is a Business Process Architect for CSC working on Identity Access and Management (IAM). She previously worked on the QIPS project for two years, and is currently working with 2020 Company, LLC on the QIMS effort. She has over six years of experience as a business process architect on various IAM projects, and was previously a member of CSC's Software Engineering Process Group specializing in evaluating software projects and developing software process improvement plans using the Software Engineering Institute's Capability Maturity Model Integration model. She has over 20 years of experience in software engineering, and received her BS in Electrical Engineering from the University of Tennessee in 1988.

CROWNWeb Tips and Tricks from the Real World (5/27/2010 @ 2pm - 3pm ET)
On May 27, 2010, The Centers for Medicare & Medicaid Services (CMS) welcomes a representative from a CROWNWeb Phase 2 submitting facility to share her experiences working with CROWNWeb, and to provide tips and tricks for effectively using the system.
Since 1988, Candy Margiera has served as the Administrator and CEO of Sarasota Physicians Dialysis Center, Inc. (SPDC) in Sarasota, FL. In addition to her work with SDPC, she has served on the Medical Review Board for ESRD Network 7, on the Board of Directors for the Florida Renal Administrators Association, and as the Southeast Regional Director for the National Renal Administrators Association.
As a key participant in both Phase 1 and Phase 2 of CROWNWeb, Candy will be sharing tips and tricks learned at SPDC as an independent facility user of CROWNWeb to meet the requirements of data entry in the system.
Vascular Access: What You Need to Know (4/29/2010 @ 2pm - 3pm ET)
When submitting Vascular Access data in CROWNWeb, what access type should you select if the patient has more than one access type for that month? On April 29, 2010, The Centers for Medicare & Medicaid Services (CMS) will share information on properly entering Vascular Access data in CROWNWeb.
Raynel Wilson has been a nephrology nurse for 29 years. In 1997, she started at The Renal Network, Inc., ESRD Network 9/10, and advanced to the position of Director of Quality Improvement in 2003. Raynel served as the project manager for the National CPM Data Collection and Report from 2000 to 2008, and has participated on many CMS/Network committee meetings related to CPMs and data collection throughout the development of CROWNWeb.
Effective, March 15, 2010 Teresa Casey began serving as the Acting Director of the Division of Quality Improvement Policy for Chronic and Ambulatory Care (DQIPCAC). Teresa brings with her a wealth of clinical expertise and knowledge of regulations, prevention, chronic kidney disease and end stage renal disease (ESRD). She has experience as a senior technical advisor for the division and as the Theme Lead for the Quality Improvement Organization Core Prevention theme. Teresa has been leading the Chronic Kidney Disease theme of the Quality Improvement Organization 9th Scope of Work, and serving as an advisor to the Quality Improvement Group’s End Stage Renal Disease (ESRD) Program. She has nine years of clinical experience in the out-patient dialysis setting with sixteen years in clinical practice as a registered dietitian.
Clinical Performance Measures (3/25/2010 @ 2pm - 3pm ET)
What are the 26 Clinical Performance Measures (CPMs)? How will they help improve the way you care for your patients? On March 25, 2010, The Centers for Medicare & Medicaid Services (CMS) joined the renal community in a discussion of the proper way to record CPM data in CROWNWeb.
Randall Webb has more than 30 years of IT and database management experience supporting public policy, epidemiology, clinical, and medical outcomes research. He is a graduate of the University of Maryland Baltimore County (BA Economics). He is the managing director of UM-KECC, which holds multiple CMS contracts related to ESRD and produces the Dialysis Facility Reports each year.
Tom Dudley has been with CMS since 2000, and has worked intimately with the development and public reporting of ESRD and Nursing Home clinical performance measures. He holds a M.S. in Nursing and Business Administration and a B.S. in Nursing from University of Maryland.
Glenda Payne, RN, MS, CNN is a nephrology nurse with more than 30 years experience in ESRD. She is part of the Survey & Certification ESRD Support Team responsible for developing resources to assist in the implementation of the ESRD Conditions for Coverage. She serves as the project officer for the Network 7 contract, as well as for the RCT special project.
CROWNWeb Batch Submission Process (2/25/2010 @ 2pm - 3pm ET)
On February 25, 2010, Norma Ofsthun from Fresenius Medical Care and The Centers for Medicare & Medicaid Services (CMS) provided an update to the renal community with the current state and future plans of the batch data submission process.
Dr. Ofsthun is the Vice President of Corporate Research at Fresenius Medical Care. She has a Ph.D. in Chemical Engineering from MIT. She oversees the Custom Reporting and Data Analysis Group and because of her familiarity with clinical data, she continually participates in CMS projects including Fistula First, eLab, Core Data Set, and CROWNWeb.
An Independent Facility Perspective: Larry Emerson, President, NRAA (1/28/2010 @ 2pm - 3pm ET)
On January 28, 2010, Larry Emerson, President of the National Renal Administrators Association (NRAA) served as the voice of dialysis facilities nationwide as he joined The Centers for Medicare & Medicaid Services (CMS) to discuss CROWNWeb from the perspective of the independent dialysis facility.
Michael Blake is the Division Director for End Stage Renal Disease (ESRD) Systems and Contract Management (DESCM). He serves as the Information Systems Security Officer for OCSQ and ESRD Systems, and obtained a Certified Information Systems Security Professional (CISSP) certification in 2008. Mr. Blake is considered to be a Subject Matter Expert (SME) in IT Security, and has served at Centers for Medicare & Medicaid Services (CMS) for over 10 years. He has a BS degree from James Madison University.
Larry Emerson, current President of the National Renal Administrators Association, has been the CEO and Administrator of the Dialysis Center of Lincoln (NE) since 2004. A Fellow of the American College of Healthcare Executives, he holds a Bachelor of Science degree in Business Administration from the University of Nebraska and a Masters in Healthcare Administration from Central Michigan University.

CROWNWeb National Release Here June 14
Are You Ready?
CMS is pleased to announce the CROWNWeb National Release will be on Thursday, June 14, 2012 at 8am ET. Starting on this date, all Medicare-certified dialysis facilities, as well as transplant and Veterans Affairs (VA) facilities that have volunteered to participate in CROWNWeb, may have their users log in to CROWNWeb to submit administrative and clinical data to CMS.
Facilities must ensure that their most recent patient and facility data are submitted to their ESRD Network by May 18, 2012. This day will serve as the deadline by which each ESRD Network must receive their facilities' data for conversion into CROWNWeb. Each dialysis facility will be responsible for ensuring that any patient and facility data that becomes available after May 18, 2012 is submitted via CROWNWeb.

Phase III Pilot Ending on April 20, 2012
CMS announced that the CROWNWeb Phase III Pilot will end on Friday, April 20, 2012. Facilities should not enter any data into CROWNWeb after this date, as data entered after this date will be permanently lost. Access to CROWNWeb will remain unavailable between the end of the Phase III Pilot and the national release. Please be sure to disable any bookmarks or direct links to the system.

CROWNWeb Phase III Extended Through April 20, 2012
On March 22, 2012, CMS announced an extension of the Phase III pilot through April 20, 2012. CMS will extend the Pilot for an additional four weeks to provide time for dialysis facilities to complete QIMS registration in preparation for the upcoming CROWNWeb National Release. CMS has also announced that all users with active QIMS accounts are welcome to log in to CROWNWeb during the remainder of the Phase III Pilot, and to enter data into the system. At the conclusion of this four-week period, CMS will provide an update to the community on the termination date of the Phase III Pilot and/or timeline for the CROWNWeb National Release.
